Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join their Prevention team.

Responsibilities of this role:
*Undertake strengths-based assessments, reviews, and support planning with adults and their carers.
*Promote well-being, independence, and safety, enabling individuals to live in the community and avoid unnecessary hospital admissions.
*Apply Care Act (2014) principles and other relevant legislation (Mental Capacity Act, Mental Health Act).
*Work collaboratively with individuals, families, and community networks to reduce isolation and build resilience.
*Use a strengths/asset-based approach in care planning and team culture.
*Liaise with health professionals, voluntary sector, and other agencies to deliver integrated support.
*Undertake safeguarding enquiries and respond to concerns about adults at risk.
*Maintain accurate case records, reports, and documentation in line with statutory requirements.
*Participate in community engagement initiatives to prevent marginalisation and promote inclusion.
*Contribute to service development and innovation, embracing change and co-production with service users.
Requirements of this role:
*Must be a Qualified Social Worker, registered with Social Work England.
*Must have minimum of 3 years post qualifying social work experience
