Job Description:
Care Co-ordinator/Senior Support Worker – Floating Support (Adults)
Salary: £28,963 per annum
Hours: 37.5 per week (including evenings, weekends & on-call)
Location: West London
Are you passionate about empowering adults to live independently? Join our team as a Care Co-ordinator/Senior Support Worker and take the lead in planning, coordinating, and monitoring person-centred support across our Floating Support service.
What you’ll do:
*Coordinate care plans and ensure timely, person-centred support delivery.
*Supervise and guide support staff to maintain high standards of care.
*Liaise with families, advocates, and health professionals to meet individual needs.
*Manage rotas, monitor service quality, and ensure compliance with CQC and Care Act standards.
*Support individuals to access community resources, build life skills, and achieve personal goals.
What we’re looking for:
*Experience in care coordination or supervising support teams.
*Strong organisational and communication skills.
*Knowledge of safeguarding, risk assessment, and person-centred planning.
*IT skills including database use to manage rotas and service user information
