Job Description:
We are seeking an experienced and compassionate Motor Neurone Disease Specialist Practitioner Nurse to lead and deliver high-quality, patient-centred care for individuals living with MND in Slough and the surrounding areas. This is a pivotal role within a multidisciplinary team, supporting patients and families across the disease trajectory in both community and clinical settings.
The post holder will provide advanced clinical expertise, coordinate care pathways, and act as a key point of contact for patients with complex neurological, respiratory, nutritional and palliative care needs.
Key Responsibilities
*Lead the care and service delivery for patients diagnosed with Motor Neurone Disease
*Undertake advanced clinical assessments and develop individualised care plans
*Provide specialist clinical support across community, clinic, and domiciliary settings
*Work collaboratively with multidisciplinary teams, including neurology, respiratory, palliative care and allied health professionals
*Support patients with progressive neurological conditions, including symptom management and advance care planning
*Deliver education, advice and emotional support to patients, families and carers
Essential Skills & Experience
*Registered Nurse with relevant professional qualification
*Proven experience leading care and service delivery for patients with Motor Neurone Disease
*Advanced clinical assessment skills
*Experience caring for patients with:
*Neurological disorders
*Respiratory failure
*Nutritional requirements and swallowing difficulties
*Progressive neurological terminal conditions
*Evidence of managing neurological disorders in a community or hospital setting
*Full UK driving licence and access to a car, with ability to travel across sites for clinics and domiciliary visits
Desirable Skills & Experience
*MSc completed in a relevant subject
*Respiratory care experience, including assessment and management of non-invasive ventilation (NIV)
*Experience running outpatient (OPD) clinics
*Recent clinical experience supporting patients with Motor Neurone Disease
Specialist Skills / Training / Experience Required
*Non-Medical Prescriber
*Knowledge and clinical experience of non-invasive ventilation (NIV)
*Clinical experience within Palliative Care
