Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join their Intermediate Care Team.

Responsibilities of this role:
*Conduct holistic assessments of individuals’ health, social, and emotional needs.
*Develop and implement person-centred care plans in collaboration with multidisciplinary teams.
*Support individuals in regaining independence through reablement and rehabilitation strategies.
*Provide rapid response to prevent unnecessary hospital admissions or long-term care placements.
*Facilitate safe and timely hospital discharges, ensuring continuity of care.
*Promote safeguarding, dignity, and advocacy for vulnerable individuals.
*Liaise effectively with health professionals, carers, and community services.
*Monitor progress and review care plans to ensure optimal outcomes.
*Maintain accurate and timely documentation in line with professional standards.
*Contribute to service development and uphold best practices in intermediate care.
Requirements of this role:
*Must be a Qualified Social Worker, registered with Social Work England.
*Must have minimum of 3 years post qualifying social work experience
