We are recruiting in Manchester for an experienced and motivated Social Care Assessor (Grade 6) to join a dynamic and community-focused service supporting residents across the city.
This role sits within an integrated model of community health and social care, working collaboratively across neighbourhood teams to help people live healthier, safer and more independent lives.
About the RoleAs a Social Care Assessor, you’ll play a vital part in delivering high-quality support to vulnerable residents. You will manage assessments, identify risks, create person-centred plans, and work alongside a range of statutory and voluntary services to make a meaningful impact in the community.
Key Responsibilities*Build effective, supportive relationships with service users to help challenge patterns of behaviour and promote positive change.
*Complete high-quality assessments that identify needs, risks, and appropriate support pathways.
*Provide tailored advice and guidance enabling service users to engage with local and national support services.
*Monitor and review plans to ensure service users remain on track to meet agreed outcomes.
*Work collaboratively with colleagues, partner agencies, and community organisations to reduce vulnerability and improve independence.
*Promote the service across agencies and contribute to new initiatives supporting Manchester residents.
*Maintain accurate and timely records, ensuring all systems and processes support effective performance management.
*Some roles at this level may involve management duties such as supervising staff or leading virtual project teams.
*Demonstrate continuous professional development and support service improvements.
