We are seeking an experienced Social Worker to join our dedicated team, focusing on Mental Health and Autism. This role is based in Stockport and offers a hybrid working model. The successful candidate will be instrumental in providing expert care and support, making a significant impact in the lives of adults with mental health issues and autism.
Day-to-day of the role:*Conduct assessments and provide support planning for adults with mental health issues and autism.
*Work collaboratively with healthcare professionals to ensure a coordinated approach to patient care.
*Implement and monitor care plans, adjusting as necessary to meet the changing needs of clients.
*Provide crisis intervention as needed and participate in a rotating on-call schedule.
*Advocate for clients, ensuring they have access to the resources and support they need.
*Maintain accurate records and documentation in compliance with legal and organisational standards.
*Engage in continuous professional development to keep abreast of the latest developments in social work and mental health care.
Required Skills & Qualifications:*Qualified Social Worker with current registration.
*Proven experience in mental health and autism care within a social work context.
*Strong understanding of the legal and ethical considerations related to social work in mental health and autism.
*Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
*Strong organisational skills and the ability to manage multiple priorities.
*Commitment to delivering high-quality, person-centred care.
