Job Purpose
To ensure care leavers from diverse backgrounds receive appropriate, high-quality support to successfully transition into adulthood. The role focuses on promoting independence, wellbeing, and positive outcomes by working in partnership with families, statutory services, and third-sector organisations. The post holder will actively contribute to team and service development activities.
Key Responsibilities
*Deliver a high-quality, outcome-focused service to care leavers, supporting their transition to adulthood.
*Prepare, update, review, and implement Pathway Plans in collaboration with the supervising manager, ensuring parallel planning where required.
*Coordinate and facilitate access to relevant services, encouraging care leavers to engage with available support.
*Monitor care leavers’ progress, wellbeing, and assessed needs, ensuring plans are implemented effectively.
*Maintain accurate, timely, and up-to-date records on the electronic case management system.
*Work holistically with care leavers during key transition points, including transfers to adult or specialist services.
*Build effective partnerships with parents, carers, and professionals to promote and safeguard young people’s welfare.
*Support care leavers to access, enter, and sustain education, training, and employment opportunities.
*Work collaboratively with external organisations to remove barriers to learning and employment.
*Support care leavers to access accommodation, maintain tenancies, and meet resettlement needs.
*Ensure eligible care leavers access welfare benefits and financial entitlements.
*Contribute to and deliver workshops or training sessions to develop independence and life skills.
*Support care leavers with additional or complex needs, including mental health issues, disabilities, young parents, and immigration matters.
*Undertake assessments for care leavers re-referred post-age 21 who wish to resume education or training.
*Support care leavers in obtaining legal advice regarding immigration status and maintain links with relevant agencies, ensuring parallel planning where required.
*Ensure all case-related expenditure is authorised, recorded, monitored, and reviewed appropriately.
*Promote equality, diversity, and inclusion, considering each care leaver’s background, culture, and life experiences.
*Maintain up-to-date knowledge of relevant legislation, policy, and best practice through ongoing professional development.
*Manage an individual caseload effectively, prioritising needs and resources.
*Escalate safeguarding concerns or risks of harm to the supervising or duty manager without delay.
*Attend and actively participate in supervision, appraisals, and team meetings.
*Contribute to service development through team and organisational activities.
*Use IT systems effectively to carry out duties.
*Act as Duty Officer on a rota basis, managing emergencies when allocated workers are unavailable.
*Report concerns regarding abuse or malpractice in line with the Council’s Whistleblowing Policy.
*Work flexibly to meet service needs, including occasional out-of-hours work and out-of-borough visits.
*Comply with all relevant legislation, council policies, procedures, and health and safety requirements.
*Undertake additional duties as required to meet the changing needs of the service.
