Job Description:
Adults Senior Practitioner - Preparing for Adulthood (PfA) Team
Hackney Council is looking for an experienced Senior Practitioner to join the Preparing for Adulthood (PfA) team on a locum basis, starting immediately. This is a fantastic opportunity to support young people with disabilities and complex needs as they transition into adulthood.
Role Overview
*Manage a complex and varied caseload, including high-risk and statutory cases
*Complete Care Act assessments, contribute to EHC plans, and lead on personalised support planning
*Provide mentoring, guidance, and practice leadership for Social Workers, NQSWs, and students
*Support the management team with supervision, audits, and case allocation
*Work collaboratively with health, education, housing, and provider partners
*Produce high-quality reports for court and multi-agency meetings
*Ensure practice is fully compliant with the Care Act 2014, Children Act 1989/2004, MCA, and all statutory requirements
Requirements
*Social Work qualification (Degree/DipSW/CQSW)
*SWE registration
*Strong background in children’s disability services or adult learning disability services (PfA)
*In-depth knowledge of the Care Act, MCA, and children’s legislation
*Excellent risk assessment, analytical, and decision-making skills
*Experience as a Practice Educator or mentoring staff
*Ability to travel for home visits and work in a hybrid model
