Job Description:
We are currently working with a client in Liverpool who is looking to recruit an experienced Social Care Assessor to join their Adult Access Team.
As a Social Care Assessor, you will be responsible for assessing individuals and, where necessary, providing short-term support or reablement to promote independence. Where longer-term intervention is required, you will complete robust assessments to support a smooth transition into Locality Teams for ongoing support.
You will work with adults aged 18+ and manage a varied caseload with the support of supervision and guidance, ensuring all work is carried out in line with relevant legislation, policies, and procedures.
Key Responsibilities:

Manage and prioritise a varied caseload effectively
*
Complete Reablement, Needs, Risk, Mental Capacity, and Best Interest Assessments, as well as reviews
*
Commission appropriate services including domiciliary care, day placements, carers support, assistive technology, and minor adaptations
*
Work collaboratively within a Multi-Disciplinary Team
*
Attend daily huddles and participate in duty responsibilities
*
Represent the Adult Access Team at external meetings when required
*Apply the Care Act’s Prevent, Reduce, Delay principles to promote independence and wellbeing
Key Requirements:
*
Previous experience working within Adult Social Care
*
Experience completing strength-based assessments
*
Knowledge of relevant legislation including the Care Act
*
Ability to manage a varied caseload effectively
*
Experience working within a Multi-Disciplinary Team
*
Strong communication and report writing skills
*
Ability to promote independence and person-centred outcomes
Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: £19-£25
Working pattern - 37 hours per week, onsite.
