Job Description:
Service Coordinator
Chesterfield
Ongoing Temporary with a View to Permanent
£25,184.25 (£12.92 per hour)
Enhanced DBS with Child & Adult Barred Check Required
The Role:
Providing a Customer focused service, supporting young Care Leavers
Working in a building with a number of self-contained flats
Conduct a full needs assessment at the point of referral into the service to establish the nature of support required
Adopt a person centred and trauma informed approach to build trust with the young individuals
Support and assist young people to build resilience and overcome barriers
Maintain and update support activities, signposting changes
Ensure H&S and property standards are maintained and work effectively as a team member delivering excellent services to the Residents
Ensure your relationships with colleagues and external agencies are effective and professional and contribute to the Association’s values, culture and ethos
Direct experience working within a young person’s service is preferrable but not essential
Hours:
37.5 hours
Monday to Friday
07:30 – 15:30 and 10:00-18:00 and 12:15 – 20:15
