Qualified Social Worker, Hospital Team
Pay rate £32.55 per hour
Contract role
Vitalis are recruiting for an experienced Social Worker to work in a hospital team in Westminster Council.
Key Responsibilities:
*Triaging referrals on behalf of the Tri-Borough Hospital Discharge Team
*Completing quick assessments for residents of the Tri-Borough
*Undertaking Care Act Assessments and developing Support Plans
*Completing Safeguarding Concern Assessments (Section 42.1)
*Setting up care packages to facilitate safe discharge
*Providing information and signposting to A&E staff
Essential Requirements:
*Social Work England Registration
*Proven experience in hospital discharge work
*Strong knowledge of the Care Act and safeguarding procedures
*Ability to work in a fast-paced environment and manage competing priorities
