Our client, Brent Council, is seeking an agency Social Worker for its Reablement service.


This role will last 6 months initially but more likely up to 12 months and is paid at £32 per hour.




The roles will involve the following:
*An integrated service that delivers fast-paced responses to hospital discharges, alongside reablement and social care support through a short-term intervention model.
*Our team ethos centres on promoting and enabling independence, and we maintain strong working relationships with partners and colleagues across hubs and intermediate care services.
*Completing Care Act assessments
*Care and Support plans
*Multi-agency working
*Working in a fast-paced way, taking up to 3 allocations per week and actioning them quickly
*Visiting people out in the community – no telephone-based assessments
*Working closely with Reablement providers and getting feedback from them
*Working in a team which includes Occupational Therapists and OT assistants
*Liaising with GPs, physiotherapists etc.
*Using Mosaic and benefitting from the Magic Notes to lessen admin tasks

Requirements of the role:
*Qualified Social Worker
*Fully Social Work England registered
*Must have post-qualified experience working within a local authority adult social care setting
*Does not require specialist Reablement experience, but the ideal candidate will have worked with adults with physical disabilities and will have demonstrable experience in a fast-paced setting such as hospital discharge, front door team, duty etc.
*Comfortable being in the local area 3 days per week for visits and some office presence
