The role of the Care Co-Ordinator will be based in our Tameside Branch. Our Tameside Branch is rapidly growing and we are looking for an experience Care Co-Ordinator to join our thriving team. As a Care Co-Ordinator, you will play a vital role in ensuring the delivery of high quality personalised care services to our clients while contributing to the growth and development of our business. Your exceptional communication skills and proactive approach to business development are essential in this dynamic and rewarding position.
The role of the Care Co-Ordinator will have the following responsibilities but will not be limited to:
*Facilitating new packages of care from the point of discharge.
*Organise and optimise the care worker rotas for efficiency in travel time and staff skill matching.
*Plan and coordinate cover for holidays, training and planned absences.
*Be the point of contact and develop long last relationships with Health Care Assistants ensuring a high level of quality of care is being delivered to our clients.
* Conduct staff supervisions, appraisals and performance management of the Health Care Assistants.
*Liaise with other professional such as GPs, Commissioners and District Nurses.
*Conducting quality assurance calls and actioning any feedback that is raised.
*Maintain and update electronic rostering systems
*Be part of the on call phone schedule


Person Specification
*Minimum of 3 years experience in a similar role
*Valid driving licence and access to own vehicle
*Extremely well organised with excellent planning and prioritising ability and high attention to detail
*Experience using Access People Planner and Care Planner (desirable)
*Ability to work unsupervised
*Previous complex care in the community experience (desirable)
*Strong problem solving skills
