Job Description:
We are seeking a friendly and organized Admissions and Discharge Coordinator to join our team at a 30-bedded home specializing in acute mental illness. As the first point of contact for residents, visitors, and staff, you will play a crucial role in creating a welcoming and supportive environment.
Elizabeth House Hospital is committed to providing safe and effective care and treatment to patients to the service. The safe discharge of these patients is equally important.
The main function of this role is to take a lead on the coordination of patient’s admission and discharge to ensure a safe and seamless transition to and from hospital to home/alternative placement. This role involves the use of effective communication with all disciplines involved in the care of the patient both with the hospital and in the wider community and ensuring key administration of key tasks essential to the patient journey through the hospital to the point of discharge.
What You’ll be Doing
Collation of key admission information eg: funding paperwork, GP and discharge summaries
Confirming relevant paperwork through the patient pathway is completed in a timely manner and submitted responsively eg: changes in observation levels
Supporministration of key meetings along the patient journey eg: MDTs, CPA,s 117 and discharge planning meetings. To ensure that the all relevant stakeholders have been included
Liaising with funders to confirm funding arrangements for activities such as leave in the community
Book transport for specific activities like leave and access to the community
Support any patients in terms of PIP and UC assessments.
Liaising with external agencies, such as the police, community mental health team, housing and Social Care where required.
Managing the Discharge Checklist to ensure all tasks are undertaken in a timely manner.
Supporting the MDT to identify the nominated Care Coordinator and/or Social Worker for each patient on admission.
Ensuring effective communication with Care Coordinators regarding:discharge - dates, times confirmed destinations.
Liaising with local teams to confirm appropriate transportation has been booked and confirmed. Including Estimated time of arrival, ensuring that the relevant unit is aware of this and that the appropriate documentation is ready to be sent with the patient {in collaboration with the MHA administrator}.
Ensure that invitations are sent to relevant agencies for participation in meetings such as a representative from Social Services or the individuals Community Team.
Using good working relationships with the ward staff facilitate assessors to speak with the relevant person on the unit for the purpose of completing care assessrnents.

• Educated to a minimum of HeaIth and Social Care Level 3.
• Evidence of CPD
• Excellent communication skills and adaptability to differing situations.
• Ability to work autonomously and to work within the MDT.
• Ability to effectively work t6 limited deadlines.
• Patient centred approach to problem solving
