About the RoleJoin Islington’s Integrated Hospital Team as a Social Worker and play a key role in the Take Home and Settle Service, supporting individuals discharged from hospital as part of a short-term service. This exciting locum position offers £33.73 per hour, with full-time hours and the flexibility of locum work. You’ll be part of a dynamic, multi-agency team, with continuous career development and opportunities to enhance your professional expertise.
Key Responsibilities*In-Home Assessments: Provide immediate assessments for patients discharged from hospital, helping them settle into their recovery at home.
*Multi-Agency Collaboration: Work closely with other healthcare professionals, conducting joint assessments and creating comprehensive, person-centred care plans.
*Safeguarding & Welfare: Ensure the welfare and safeguarding of vulnerable adults, managing complex cases and reducing risks.
*Care Act 2014: Perform Mental Capacity Assessments and implement the Care Act 2014, ensuring the best outcomes for service users.
*Engage with Service Users: Provide advice and support to service users, their carers, and professionals, enabling individuals to live safely and independently.
*Record Keeping & Data Management: Maintain accurate records and manage data in line with Council policies.
*Training & Development: Participate in ongoing training, supervision, and team meetings to foster professional growth and meet service outcomes.
