Key Responsibilities*Community Response and Reablement Team:
*Provide information, advice, and reablement services to new community members.
*Assist individuals in maintaining their independence within their homes.
*Community Independence and Wellbeing Team:
*Deliver essential social work and occupational therapy services to individuals across diverse care settings.
*Promote community integration and support for vulnerable adults.
*Hospital Team Support:
*Work with the hospital team to deliver comprehensive 7-day social work and care services, supporting individuals, carers, and families during the hospital discharge process.
*Ensure smooth transitions from hospital to home, ensuring that individuals’ social care needs are met.
*Collaboration with Community Teams:
*Address the needs of individuals with learning disabilities and mental health challenges.
*Coordinate with healthcare and social services professionals to provide comprehensive support to service users.
*Community Engagement:
*Develop and implement strategies to build strong, supportive relationships with the local community.
*Engage with individuals and families to ensure their care needs are met while empowering them to live independently.
