This is an exciting opportunity for two enthusiastic Adults Social Workers to join the West Locality Team at Middlesbrough Borough Council. What you will do:
- Use a person-centred, conversational, and trauma-informed approach to engage with adults dealing with autism, learning disabilities, physical disabilities, acquired brain injuries, addiction, and trauma.
- Conduct strengths-based assessments in line with the Care Act 2014.
- Develop outcome-focused care and support plans to promote clients’ independence and wellbeing.
- Engage closely with partners such as NHS Trusts and local services to provide integrated care.
- Deliver timely interventions in a brisk yet collaborative environment.

What we’re looking for:
- A qualified Social Worker with Social Work England registration.
- Demonstrable skills in assessment and care planning.
- The ability to work proactively in a bustling and fast-paced setting.
- Enthusiastic about multi-agency working to enhance client outcomes.
- Capable of meeting travel requirements for client visits and meetings.
