Social Worker – Adults Hospital Team
Location: East Lothian
Contract: Temporary, Full-time
Pay: £40 per hour
Working Pattern: Hybrid working available | Option for condensed 4-day work week
Seven Resourcing are looking for an experienced Social Worker to join a dedicated Adults Hospital Team in East Lothian. This is a full-time temporary position offering a competitive hourly rate, flexible hybrid working, and the option of a condensed 4-day working week to support work-life balance.
In this role, you will support adults within a hospital setting, helping to facilitate safe and timely discharges while ensuring individuals receive the care and support they need.
Key Responsibilities of the Social Worker:
*Complete assessments to support hospital discharge planning
*Work collaboratively with multidisciplinary teams to coordinate care
*Develop and review care packages in line with individual needs
*Ensure compliance with relevant legislation and safeguarding procedures
*Maintain accurate and up-to-date case records
*Promote independence and positive outcomes for service users
Applicant Requirements:
*Qualified Social Worker (BA/MA/BSc/MSc in Social Work, CQSW, or DipSW)
*Registered with the SSSC
*Minimum 2 years’ post-qualifying experience within adult social care
*Previous hospital discharge or acute setting experience preferred
*Enhanced DBS on the update service (or willingness to obtain)
*Full UK driving licence and access to a vehicle
*Ability to work across East Lothian as required
