What You Will Do
*Conduct comprehensive functional assessments and develop personalised, goal-oriented reablement plans.
*Collaborate with reablement carers, physiotherapists, and other multidisciplinary professionals to deliver holistic care.
*Provide clinical coaching on rehabilitative techniques to support client progress.
*Recommend, fit, and guide clients on low-level equipment such as mobility aids and minor adaptations for accessibility.
*Manage your caseload independently, including travelling to peripheral locations to collect and deliver equipment.
Additional Requirements
*HCPC registration is mandatory.
*Car driver essential due to travel requirements.
