Job Description:
Key responsibilities*Deliver high-quality community nursing care to patients in their homes and community settings
*Carry out assessments, care planning and clinical interventions as a Band 5 nurse
*Manage and prioritise a caseload of community patients
*Administer medications and carry out clinical procedures in line with NMC standards
*Work collaboratively with the wider neighbourhood MDT including GPs, social care and therapy teams
*Complete accurate and timely clinical documentation
*Escalate concerns regarding patient deterioration appropriately
*Contribute to the safe and effective running of the community nursing service
Essential:
*Registered Nurse with active NMC pin
*Experience in community or district nursing (desirable but not essential)
*Ability to manage own caseload and work autonomously
*Full UK driving licence and access to a vehicle
*Good communication and documentation skills
*Enhanced DBS check required
