Full job descriptionWe are seeking an organised, proactive, and experienced Part-Time Care Coordinator to join Feldon Care. The successful candidate will play a key role in coordinating care services, supporting care staff, and ensuring the delivery of high-quality, person-centred care.
Key Responsibilities:
*Coordinating and scheduling care visits efficiently
*Managing staff rotas and ensuring adequate care coverage
*Liaising with clients, families, healthcare professionals, and care staff
*Conducting care reviews, assessments, and care plan updates
*Monitoring service quality and supporting compliance with CQC standards
*Assisting with recruitment, onboarding, and staff support
*Maintaining accurate records and care documentation
*Responding to day-to-day operational and service delivery needs
Requirements:
*Previous experience as a Care Coordinator within domiciliary care or a similar health and social care setting
*Strong organisational and time-management skills
*Excellent communication and interpersonal skills
*Good understanding of care planning, safeguarding, and compliance requirements
*Ability to work independently and manage multiple priorities
*Competent in using Microsoft Office and care management systems
*Up-to-date Mandatory Training certificates
*Right to work in the UK
*Enhanced DBS (or willingness to obtain one)
*Full UK driving licence and access to a vehicle desirable but not essential
