We are looking for experienced and passionate Complex Care Nurse Consultants to join our growing Clinical & Complex Care at Home Team. This is an exciting opportunity for skilled nurses who are dedicated to delivering high-quality, person-centred care to individuals with complex healthcare needs in their own homes.
As a Complex Care Nurse Consultant, you will play a pivotal role in ensuring the highest standards of clinical care, supporting service users to live safely and independently while leading and developing care teams.
Key Responsibilities
*Deliver and oversee complex clinical care for service users within their own homes.
*Complete comprehensive clinical assessments and develop person-centred care plans.
*Provide clinical leadership, supervision, and support to care staff.
*Undertake competency assessments and clinical training for support workers.
*Monitor and review care packages to ensure safe and effective service delivery.
*Work collaboratively with families, multidisciplinary teams, and healthcare professionals.
*Ensure compliance with clinical governance, safeguarding, and regulatory requirements.
*Maintain accurate clinical records and documentation.
Requirements
To be successful in this role, you will have:
*Current NMC Registration (Adult, Children’s or Learning Disabilities Nurse).
*Previous experience supporting individuals with complex healthcare needs.
*Strong knowledge and practical experience of complex clinical interventions, including:
*PEG feeding
*Tracheostomy care
*Stoma care
*Ventilation support (desirable)
*Catheter care
*Medication administration
*Excellent clinical assessment and decision-making skills.
*Strong leadership, communication, and organisational abilities.
*Full UK driving licence and access to a vehicle (desirable, depending on location).
