We are seeking a dedicated Mental Health Social Worker to join our Front Door team at Salford Civic. This role involves conducting visits and assessments in support of the contact team, offering a vital service in the community. Hybrid working options are available, allowing for up to two days of work from home per week.
Day-to-day of the role:
*Conduct comprehensive assessments of clients’ mental health needs.
*Collaborate closely with the contact team to provide timely and effective support.
*Make home visits to clients as part of the assessment and support process.
*Develop and implement tailored care plans based on assessment findings.
*Maintain accurate and detailed records of assessments, interventions, and outcomes.
*Participate in regular team meetings to discuss cases and update on client progress.
*Provide crisis intervention as needed.
Required Skills & Qualifications:
*Qualified Social Worker with a current registration.
*Experience in mental health settings, particularly in conducting assessments and home visits.
*Strong understanding of mental health issues and the social factors that affect mental health.
*Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
*Ability to work independently and as part of a multidisciplinary team.
*Proficient in maintaining accurate and detailed records.
