Description:
We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.


Key Responsibilities:
*Manage a caseload of up to 35 patients with severe and enduring mental health conditions
*Act as Care Coordinator in accordance with the Care Programme Approach (CPA)
*Complete comprehensive mental health assessments and risk assessments
*Develop, implement, and review person-centred care plans
*Work collaboratively with the multidisciplinary team and external agencies
*Provide therapeutic interventions and ongoing support to service users
*Maintain accurate clinical records and documentation in line with NHS policies
*Promote recovery, independence, and positive mental health outcomes


Requirements:
*Valid NMC Registration (Registered Mental Health Nurse)
*Previous Community Psychiatric Nursing (CPN) experience
*Experience managing a community mental health caseload
*Knowledge of CPA, risk assessment, and safeguarding procedures
*Up-to-date mandatory training
*Enhanced DBS
*Available to start ASAP
