Job overviewAn exciting opportunity has arisen to join Adult Community Services Division as a Senior Practice Governance Facilitator. You will support the delivery of patient safety, practice governance and quality improvement across a large and diverse community services division. You will work closely with the Head of Nursing and in collaboration with the Divisional Director, Clinical Director, Heads of Service, Professional Leads and operational teams to ensure learning from incidents, complaints, audits and service user feedback is translated into meaningful improvements in care.
This is a varied and rewarding role for an individual with experience in governance, patient safety, audit or quality improvement who is passionate about supporting services to deliver safe, effective and compassionate care. You will provide governance support and advice, coordinate quality improvement initiatives, support Patient Safety Incident Response Framework (PSIRF) processes, monitor improvement plans and contribute to governance assurance across the Division.

Main duties of the jobThe Senior Practice Governance Facilitator will support the delivery of patient safety, governance and quality improvement across Adult Community Services Division. Working closely with the Head of Nursing and in collaboration with the Divisional Director, Clinical Director, Heads of Service and Professional Leads, the post holder will:
*Support the implementation of governance, patient safety and quality improvement priorities across the Division.
*Coordinate learning from incidents, complaints, audits and Patient Safety Incident Response Framework (PSIRF) learning responses, ensuring improvements are implemented and embedded.
*Monitor governance and improvement action plans, providing reports and assurance through governance structures.
*Analyse quality and safety data to identify trends, risks and opportunities for improvement.
*Support teams to deliver quality improvement projects, audits and service developments.
*Provide specialist advice and support on governance, patient safety and quality improvement matters.
*Deliver training as required and promote the sharing of learning and best practice across services

Working closely with the Head of Nursing and in collaboration with the Divisional Director, Clinical Director, Heads of Service, Professional Leads and operational teams, the post holder will support services to embed learning, improve practice and provide assurance on the quality and safety of care.
The role will coordinate learning from incidents, complaints, audits, quality assurance visits and Patient Safety Incident Response Framework (PSIRF) learning responses, ensuring actions are implemented and improvements embedded into everyday practice. The post holder will monitor governance and quality improvement action plans, analyse quality and safety data, identify trends and risks, and support services to deliver continuous improvement initiatives.
Key responsibilities include supporting divisional governance processes, facilitating clinical audits, preparing reports and assurance updates, providing specialist advice on governance and quality improvement matters, and supporting teams to respond to regulatory and quality assurance requirements. The post holder will also contribute to staff training, promote the sharing of learning and best practice, and support the involvement of service users and carers in service improvement activities.
Car driver essential as a majority of the posts are community based, the post holder will need a full valid driving licence and access to a car to use regularly for business purposes - including business insurance (unless you have a disability as defined by the Equality Act 2010).
