Full job descriptionOur client, Shropshire Council, is seeking an agency social worker for its Acute Hospital service.


This is a temporary ongoing role paid @ £31.97 per hour.


Details of the role:
*The Acute Hospital Social Worker supports safe and timely discharge for people with complex care and support needs.
*Complete Care Act Assessments.
*Undertake Mental Capacity Assessments and Best Interests decisions.
*Arrange Pathway 3 placements where long-term care is required.
*Work with individuals, families, and partner agencies to ensure appropriate care and support is in place.
*Respond to referrals, undertake assessments, and support timely hospital discharges as part of the Hospital Discharge Team.
*The role is primarily based at Royal Shrewsbury Hospital and requires regular on-site attendance.
*Workspace is available at Guildhall between visits if required.
*Hybrid working may be considered, subject to the requirements of the role.
*Occasional weekend working may be required between 9:00am and 1:00pm on Saturdays and Sundays, subject to further discussion.
*No fixed office-based days; however, flexibility is dependent on maintaining service delivery requirements.

Requirements of the role:
*Qualified Social Worker
*Fully Social Work England registered
*Must have significant post-qualified experience in adult social care
*You will ideally have experience of hospital-based social work or knowledge in this area.
