We have an exciting opportunity for an enthusiastic and motivated individual to join the Arranging Support Hospital Discharge Team. This role offers the chance to develop your knowledge and experience within a busy and rewarding service, working closely with Health colleagues, Locality Teams, providers, and other key partners to support safe and timely hospital discharges.
As a Coordinator, you will play a vital role in ensuring people receive the right care and support at the right time. You will be responsible for managing and prioritising referrals, sourcing placements, negotiating with providers to achieve best value, and supporting individuals and their representatives to understand the choices available to them and any associated costs.
The role also involves working with individuals and families to explain care funding arrangements and undertaking light-touch financial assessments where required. Strong communication, organisational and problem-solving skills are essential, as is a commitment to delivering high-quality, person-centred outcomes.
